The Physical Physical

6-Month Movement Checkup

Just like you see your doctor for an annual physical and your dentist twice a year, your movement and joints deserve regular checkups too. Most “physicals” barely touch how you actually move, lift, or live day to day.

At your last annual physical, was there anything actually physical about it?

The Physical Physical is a 6‑month wellness checkup designed to catch issues early and keep you ahead of problems. During this session, we’ll:

  • Assess your functional movement patterns and individual joint function
  • Review your current training, activity, and recovery habits
  • Identify nagging aches, pains, or “wonky” movement patterns you may have been ignoring
  • Outline specific exercises or adjustments to keep you progressing safely
  • When appropriate, incorporate key objective measures like cardiovascular capacity, recovery, and body composition to give you a clearer performance baseline

Think of it as a tune‑up for your body so you’re not metaphorically driving around with your check‑engine light on.

If you want to stay proactive instead of reactive, this session helps you course‑correct before small issues become bigger problems.

For people who need flexibility or want to experience my approach before committing to a program, I offer standalone concierge sessions in different time durations. You can choose the session length that best fits your goals, schedule, and current needs.

Additional visit lengths and custom personal training plans (1–3 sessions per week) are available and can be tailored during your consultation.

This is a plain‑language overview. You’ll receive a full Financial Agreement and, if applicable, a Medicare ABN to review and sign before starting care.

Sessions and Payment

  • Sessions should be used within 12 months of purchase.
  • Payment is expected at the time of service unless we’ve made other arrangements in writing.

Out‑of‑Network & Reimbursement

  • Utility Health, PLLC is out‑of‑network with all commercial health plans.
  • If you have out‑of‑network benefits, we can provide an invoice for you to submit to your insurer at your discretion.
  • You’re responsible for checking your own benefits, deductibles, and any required referrals or pre‑authorizations.

Wellness & Fitness Services

  • Most commercial plans and Medicare do not cover wellness or fitness services.
  • We’ll provide a receipt upon request; you’re responsible for making sure use of HSA/HRA/FSA funds complies with IRS and plan rules.

Packages & Refunds

  • Single Concierge Sessions are non-refundable once delivered.
  • If after the first 2 sessions you genuinely feel this isn’t a fit, tell me and I’ll refund the remaining program fees or convert them to a single‑session rate, your choice.
  • 12-Week Packages – If requested in writing, full refund if no sessions used yet, or converted to credit for other services.
  • 12-Week Packages – If requested in writing, charge for sessions delivered at the package per‑session rate, refund for the unused portion of the package or converted to credit for other services. 
  • Annual SubscriptionsCharges already processed are non‑refundable. 
  • Annual SubscriptionsUnused sessions in a billing cycle can be rolled forward once (up to 60 days), then expire if unused

HSA/HRA/FSA

  • We accept these for medically necessary services.
  • For wellness or fitness services, consult your plan and tax advisor; we cannot guarantee eligibility.

Medicare & Medicare Advantage

  • We do not bill Medicare or Medicare Advantage through Utility Health, PLLC, and services provided here are not intended to be submitted to Medicare or MAPs.
  • If you wish to use Medicare or MAP benefits, you should seek care from a provider who bills those plans directly.
  • As a condition of being seen here, you agree to pay privately and not submit our invoices to Medicare or Medicare Advantage for reimbursement.

Cancellation Policy

  • We require 24 hours’ notice to cancel or reschedule.
  • Late cancellations or no‑shows may be charged up to the full session fee.

Privacy & Records

  • If you pay privately at the time of service, we will not disclose your records to your health plan or Medicare unless you later authorize us in writing.

For full details, including appeals and service termination policies, please refer to the Utility Health Financial Agreement you’ll receive before beginning care.

This summary does not replace the full Financial Agreement, which governs your relationship with Utility Health, PLLC.
